Prior to your consultation with A/Prof Reid, please download and complete the forms below.  The forms can either be emailed to us prior to your appointment, or you can bring them to your appointment. If you are unable to complete the forms prior to your appointment, please arrive 30 minutes early to your scheduled appointment time in order to complete the forms in our office. 

If you are having a pelvic ultrasound or endometriosis scan, please ensure you bring a signed referral form from your GP or Specialist to your ultrasound appointment. The ultrasound referral form can be downloaded below.
